New Permit Request

To begin using radiation sources at the University of Utah, the following must be completed: Complete Permit Application. All Radiation Workers that are to be authorized on the permit must complete the appropriate training. Radiation Workers on this permit may or may not be required to wear radiation dosimetry. If a radiation worker is issued radiation dosimetry, they are expected to wear and manage their dosimeter(s) in accordance with their training.
