Report Unwanted Event of Near Miss

Any “Unwanted Event” (e.g. medical injury, floor and/or personal contamination, accident resulting in damage to instruments or property, etc.) or “Near Miss” (i.e. an incident/accident in which these occurrences were barely avoided) shall be reported as directed by the University of Utah’s Emergency Response Guide.

If radiation is involved with the Unwanted Event or Near Miss, ensure that Radiation Safety is contacted even if the first point of contact may be to another group or individual. Contact Radiation Safety at 801-481-6141 or click on the Report Incident button linked below for online notification.
